The 12CF1-1PW148-30R is a power adapter cable with a 16-pin 12VHPWR connector (12 power pins and 4 sense pins) on one end and four PCIe 8-pin connectors on the other. The 16-pin connector carries the NVIDIA logo.

It is meant for graphics cards with a 12VHPWR power socket when the power supply has standard PCIe 8-pin leads and no dedicated 12VHPWR cable. Each 8-pin connector goes to a PCIe lead of the power supply, and the 16-pin connector goes to the graphics card.

Push the 16-pin connector all the way in until the latch clicks, and avoid bending the cable sharply right next to the connector. The unit is new, in bulk packaging (bag).
